Bus pass requests must be written by a legal parent or guardian and turned in to the main office by 1:00 PM.

This information may also be submitted by email to firstname.lastname@example.org by 1:00 PM.
Requests must include:
- Your student’s name
- Your student’s ID Number
- First and last name of the child your student is riding with
- Bus number of the bus your student will be riding on(Find this information HERE, click on Riding the Bus, Bus Planner, then Transportation Eligibility)
- Location of drop off
- Date transportation requested
- Parent Signature
Passes may be picked up in the main office at the END of the school day.
*Please note that Preston Middle School may issue five (5) bus passes per student per school year. All other bus transportation requests must be made through the PSD Transportation Department.
